Our Aims

We are passionate about General Practice and building a sustainable GP Network model as the foundation of a “place based” integrated care community covering 30-50,000 patients. We believe that our merged, transformed and high quality General practice organisation – Middlewood – will not only provide a viable and efficient model of general practice for the 21st century but will also allow a flourishing new model of care across the community we serve, incorporating the skills of community, mental health, social care and 3rd sector partners.

We plan to work within small practice-based teams, supported by a larger resilient and diverse accountable organisation. We are creating a ‘Practice for the Future’, that is safe, sustainable and resilient and that works for our patients and communities, and for our staff. It fully aligns to the GP Forward View and the models needed to transform the NHS and embrace and unlock the opportunities and potential of our communities.
